You need to know what the “Community Diagnosis” is before you pick resources. Use AHRQ resources and Healthy People 2030 SDOH data tools to assess the most pressing community needs, such as food insecurity or lack of access to transportation .

To achieve a “Distinguished” grade, choose resources from a variety of categories:
